With our deepest sorrow and broken hearts, we announce Jacob Reilly Johnson, born March 25, 1997, suddenly passed away on November 27, 2022.

Jake will be missed by so many. He is survived by his mom and stepdad, Heidi Johnson & Damion Jarger; dad and stepmom, Chad & Theresa Johnson; grandparents, Phil & Bonnie Johnson; and Gayle (Grams) Putnam.

He loved his aunts, uncles, and all his cousins. He had a very special bond with his cousin Spencer Putnam. They were best friends, more like brothers.

He was passionate about soccer, snowboarding, and shoes. Especially shoes!

We find comfort in knowing he is in heaven with his brother, C.J. Johnson (his soul mate), and other friends and family who are walking him through his next journey.

Jake's kindness, silliness, laughter, and his big, beautiful smile and soul will be deeply missed.
